KINDERGARTEN REGISTRATION PROCEDURES AND REQUIREMENTS

STEP 1
Complete Online Registration

Click on Kindergarten Preregistration and complete the form.
Upload the following registration documents to our student management system, Realtime:

PROOF OF BIRTH

PROVIDE 1 FROM THIS SECTION

Child’s Birth Certificate
Child’s Valid Passport

PROOF OF RESIDENCY #1

PROVIDE 1 FROM THIS SECTION

Deed
Current Lease Agreement
Mortgage Statement
Current Property Tax Bill

PROOF OF RESIDENCY #2

PROVIDE 2 FROM THIS SECTION
Must show name and Allendale address

Driver’s License
Bank Statement
Credit Card Statement
Utility Bill (one of the following: phone, cable, gas, electric, or water bill)

STEP 2

Required Registration Forms

Affidavit of Residency. Both residential parents must be present to sign the document. A notarized hard copy must be returned to us.
Child’s 4-Year-Old Physical (or newer)
Child’s Immunization History
Preschool Experience Inventory (to be completed by the child’s preschool teacher & returned to us)
Complete the Home Language Survey (need not be handed in)
Complete the Kindergarten Parent Input Form (need not be handed in)

*Children must reside in Allendale full-time to attend Hillside School. Review our Residency Policy and Regulation 5111: Eligibility of Resident/Nonresident Students regarding residency requirements, available on our website.
